Freedom At Home is seeking a compassionate Physical Therapist (PT) to join our home health team in Oakland County, Michigan. You will evaluate patients in their homes, develop personalized treatment plans, and deliver skilled therapy with emphasis on independence and safety.

Responsibilities include completing OASIS and clinical documentation, collaborating with physicians and nurses, and educating patients and caregivers on exercises. A flexible schedule and supportive leadership await you.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Michigan Physical Therapist license.
• Current CPR certification.
• Minimum of 2 years of PT experience preferred.
• Home Health experience is a plus but not required.
• Valid driver's license, reliable transportation, and auto insurance.
