Job summary

Freedom at Home is seeking a compassionate Occupational Therapist to join our growing home health team in Southeast Michigan. The role offers excellent pay, paid mileage, flexible compensation, and the option for salary or per-visit pay with a sign-on bonus.

You will evaluate patients in their homes, develop individualized treatment plans, ensure home safety, educate patients and caregivers, and collaborate with a multidisciplinary team to achieve optimal outcomes.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Michigan Occupational Therapist license.
• Current CPR certification.
• Minimum of 2 years of Occupational Therapy experience preferred.
• Home Health experience is a plus but not required.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans.
• Provide skilled occupational therapy services to improve independence and quality of life.
• Assess home safety and recommend adaptive equipment or modifications.
• Educate patients and caregivers on therapeutic exercises, self-care techniques, and safe use of assistive devices.
• Complete OASIS assessments and clinical documentation accurately and on time.
• Collaborate with physicians, nurses, and other healthcare professionals to achieve the best patient outcomes.
• Monitor patient progress and adjust treatment plans as appropriate.
• Promote safe, compassionate, high-quality patient care.
