Job summary

Freedom at Home is seeking a compassionate Physical Therapist to join our home health team in Michigan. The role emphasizes patient-centered care, education, and collaboration with a multidisciplinary team to promote independence and safe mobility in patients' homes.

We offer competitive hourly pay, per-visit compensation, mileage reimbursement, and flexible scheduling. A sign-on bonus is provided to support the transition and start of caring for patients in their homes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Michigan Physical Therapist license.
• Current CPR certification.
• Minimum of 1 year of PT experience.
• Home Health experience is a plus but not required.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans.
• Provide skilled physical therapy services.
• Educate patients and caregivers on exercises, safety, and mobility.
• Complete OASIS and clinical documentation in a timely manner.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Monitor patient progress and adjust treatment plans as needed.
• Promote safe, high-quality patient care and excellent outcomes.
