Job summary

InHome Therapy is seeking a licensed Physical Therapist to provide in-home care for adult and geriatric patients in and around Plainfield. You will deliver PT services with a flexible schedule and a stable caseload, backed by a supportive team and reimbursement for mileage.

Qualifications

• Active physical therapy license in good standing.
• BLS CPR certification.
• Reliable transportation to patient homes.
• Comfort with using email, texting, and EMR systems (training provided).

Responsibilities

• Deliver in-home physical therapy to adults and geriatric patients.
• Evaluate mobility, strength, and function to set goals.
• Implement rehab and therapeutic exercises.
• Educate patients and families on recovery plans and prevention.
• Maintain accurate home health documentation per Medicare and agency standards.

• IHT PT Clinical Excellence Bonus - earn additional weekly pay based on visit complexity and volume
• The evaluation visit rate for this role ranges from $65 - $85 per visit

Key Responsibilities

*This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some full-time roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO.
