Job summary

InHome Therapy is seeking a licensed Physical Therapist to provide in-home care for adult and geriatric patients in and around Plainfield. You will deliver PT services with a flexible schedule and a stable caseload, backed by a supportive team and reimbursement for mileage.

The role offers per-visit compensation, ongoing training, and opportunities for professional growth. Candidates must have reliable transportation and be comfortable with EMR systems used by the agency.

Qualifications

• Active physical therapy license in good standing.
• BLS CPR certification.
• Reliable transportation to patient homes.
• Comfort with using email, texting, and EMR systems (training provided).

Responsibilities

• Deliver in-home physical therapy to adults and geriatric patients.
• Evaluate mobility, strength, and function to set goals.
• Implement rehab and therapeutic exercises.
• Educate patients and families on recovery plans and prevention.
• Maintain accurate home health documentation per Medicare and agency standards.
