Job summary

Cliveden Nursing & Rehab Center in Philadelphia, PA, is seeking a Full-Time Physical Therapist. You will join an in-house rehab team that supports growth, mentorship for new graduates, and a collaborative work culture. The role offers competitive compensation and a comprehensive benefits package.

Responsibilities include evaluating residents, creating individualized plans, educating families, and ensuring accurate documentation and billing. A PA PT license (or eligibility) is required.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Pennsylvania (or eligibility to obtain).
• New graduates are encouraged to apply.
• Strong communication and interpersonal skills.

Responsibilities

• Evaluate residents and patients and develop individualized treatment plans.
• Provide skilled physical therapy services in accordance with physician orders and established plans of care.
• Educate patients, families, and caregivers regarding treatment goals and therapeutic interventions.
• Monitor patient progress and modify treatment plans as appropriate.
• Complete timely and accurate documentation in accordance with company policies and state and federal regulations.
• Communicate patient status and progress with physicians, nursing staff, and interdisciplinary team members.
• Assist with scheduling and accurately complete daily treatment charges.
