Job summary

AdviniaCare Pawtucket is seeking a compassionate Physical Therapist to join our interdisciplinary rehabilitation team. You will design and implement personalized treatment plans to promote independence, safety, and quality of life for residents in Pawtucket, RI.

As a licensed PT, you will work closely with colleagues to assess needs, modify plans, educate residents and families, and document progress. A $5,000 sign-on bonus accompanies this full-time role, effective 8/28/2026.

Qualifications

• Licensed Physical Therapist required.
• Active licensure in Rhode Island preferred.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals.
• Implement and modify treatment plans to address physical, cognitive, and emotional needs.
• Collaborate with interdisciplinary team to ensure coordination of care.
• Educate residents and families on therapeutic exercises and techniques.
• Monitor and report on resident progress and outcomes.
• Maintain accurate and up-to-date records of patient care.
