Job summary

AdviniaCare Pawtucket is seeking a compassionate Physical Therapist to join our interdisciplinary team in Pawtucket, RI. You will design and implement personalized treatment plans to promote independence, safety, and quality of life for residents.

As a Licensed Physical Therapist, you will collaborate with rehabilitation staff, educate residents and families on exercises, monitor progress, and maintain accurate records in a supportive, patient-centered environment.

Qualifications

• Licensed Physical Therapist credential required.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with interdisciplinary team to ensure coordination of care
• Educate residents and their families on therapeutic exercises and techniques
• Monitor and report on resident progress and outcomes
• Maintain accurate and up-to-date records of patient care
