Job summary

Concourse Rehab & Nursing Center in Bronx, NY, seeks a Physical Therapist to evaluate residents, create individualized treatment plans, and deliver hands-on therapy to promote recovery and independence.

You will join a collaborative interdisciplinary team in a sub-acute/long-term care setting, working full-time, in-person 8-hour shifts, with a competitive salary and a $2,000 sign-on bonus.

Qualifications

• Must have degree in Physical Therapy from an accredited program.
• Active NY State PT license required.
• BLS certification required.
• Experience in sub-acute rehab or long-term care preferred; new graduates welcome.
• Strong communication, patient education, and documentation skills.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop and implement evidence-based treatment plans.
• Provide therapeutic exercises, mobility training, manual therapy, and restorative treatments.
• Evaluate, recommend, and assist with orthotic and prosthetic devices.
• Monitor patient progress and adjust treatment plans.
• Educate residents and families on home exercise programs and recovery goals.
• Supervise Physical Therapy Assistants and collaborate with nursing and rehab staff.
• Maintain accurate and timely clinical documentation in compliance with regulations.
