Job summary

Live Well Homecare is seeking a licensed Physical Therapist to provide comprehensive in-facility rehabilitation services reporting to the Director of Rehab. The PT will assess patients, develop personalized recovery plans, and deliver therapy to restore mobility and independence.

You will educate patients and caregivers, collaborate with colleagues, and contribute to a patient-centered rehabilitation program.

Qualifications

  • Must hold a valid Physical Therapy license in the applicable state or be eligible.
  • Master's degree in Physical Therapy or related field, or equivalent experience.
  • Minimum of 5 years of progressive PT experience if holding a Bachelor's degree.
  • Ability to adapt to diverse patient needs and work in various settings.
  • Strong collaboration, communication, and teaching skills.

Job description

Role Overview:

Responsible for providing comprehensive physical therapy services in an in-facility setting, reporting to the Director of Rehab. The Physical Therapist assesses patient needs, develops personalized recovery plans, and delivers therapy to restore and enhance physical abilities. They serve as both healer and educator, training patients and caregivers to promote independence and mobility.



Key Responsibilities:

  • Collaborate with colleagues and make referrals as needed

  • Educate patients and caregivers for sustained independence

  • Valid Physical Therapy license in applicable state(s) or proof of eligibility
