Job summary

Vasona Creek Healthcare Center in the Los Gatos/South Bay area is seeking a dedicated Physical Therapist (PT) to join our short-term rehabilitation team. You will work in a brand-new therapy space with advanced equipment and a multidisciplinary team to help patients regain function and independence.

Qualifications

• Current California Physical Therapist license required.
• Experience in skilled nursing or post-acute rehab settings preferred.
• Strong clinical reasoning and communication skills.
• Commitment to evidence-based, patient-centered care.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized, evidence-based treatment plans.
• Provide skilled therapy services to short-term rehabilitation and long-term care residents.
• Treat patients recovering from joint replacements, cardiac events, strokes, and other medical conditions.
• Utilize specialized rehabilitation programs including Acute Neuro Rehabilitation and Pain Management.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with PTAs, OTs, SLPs, nursing, and other team members.
• Educate patients and families regarding treatment plans and activities for progress.
• Promote safe mobility, independence, and effective discharge planning.
