Job summary

Signature HealthCARE seeks a licensed Physical Therapist to join Signature Rehab for residents across Signature HealthCARE facilities in Lexington, Kentucky. The role focuses on rehabilitation within a comprehensive care model, enabling full recovery and timely return to daily activities.

Responsibilities include delivering PT services, collaborating with the clinical team, and supporting both inpatient and outpatient rehab programs with a patient-centered approach.

Qualifications

• Graduate of accredited physical therapy program with a bachelor’s degree.
• Valid Physical Therapy license in the state where services are rendered.
• Weekend availability required.

Responsibilities

• Provide rehabilitative therapy to residents and patients within the Signature HealthCARE network.
• Collaborate with physicians and other clinicians to develop and implement care plans.
• Support outpatient rehabilitation services through Signature Rehab and nursing home programs.

What You Need To Make a Difference

We are recruiting for a Physical Therapist to join our team. Requirements for consideration include:

• Graduate of accredited program of physical therapy, culminating in a minimum of a Bachelor's of Science Degree
• A Valid Physical Therapy license in the state where services are rendered.
