Job summary

TEEMA is seeking a licensed Physical Therapist for a SNF setting in Minnesota on a contract basis. The role focuses on assessing residents, developing individualized treatment plans, and delivering interventions to improve strength, balance, and mobility.

You will monitor progress, document outcomes, and collaborate with an interdisciplinary team to optimize care plans, with flexible extension potential and competitive pay.

Qualifications

• Current Physical Therapist licensure.
• 1 year of experience.
• Experience in a SNF or geriatric care setting is a plus.
• Excellent communication and interpersonal skills.
• Covid-19 Vaccine and Flu Vaccine status required.

Responsibilities

• Assess and evaluate residents’ physical therapy needs and develop individualized treatment plans.
