LiveWell is seeking a licensed Physical Therapist to join our in-house rehabilitation team. You will evaluate residents and patients, develop individualized treatment plans, and implement interventions to restore mobility and independence.

This collaborative, patient-centered environment offers ongoing clinical training, supportive colleagues, and growth opportunities. Experience in skilled nursing or long-term care is valued but not required.

Qualifications

  • Current Physical Therapist license (state-specific).
  • Graduate of an accredited Physical Therapy program.
  • Strong clinical assessment and documentation skills.
  • Excellent communication and teamwork abilities.
  • Experience in skilled nursing or long-term care preferred (but not required).

Responsibilities

  • Evaluate patients and develop individualized treatment plans
  • Plan and administer therapeutic interventions to relieve pain and improve mobility
  • Restore or enhance functional movement and independence
  • Ensure all treatments follow the established plan of care
  • Monitor and document patient progress and response to treatment

Position Overview

As a Physical Therapist, you will evaluate, develop, and implement individualized treatment plans for residents and patients with temporary or long-term physical conditions. You will play a key role in restoring function, promoting healing, and improving overall quality of life.
