Lancaster Nursing and Rehabilitation Center is seeking a licensed Physical Therapist to join its in-house rehab team. You will evaluate, develop, and implement individualized treatment plans for residents requiring physical therapy, aiming to restore function and improve quality of life.

You will work in a collaborative, interdisciplinary environment with ongoing clinical training and development opportunities.

Formación

• Current Physical Therapist license (state-specific).
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment and documentation skills.
• Excellent communication and teamwork abilities.
• Experience in skilled nursing or long-term care preferred (but not required).

Responsabilidades

• Evaluate patients and develop individualized treatment plans.
• Plan and administer therapeutic interventions to relieve pain and improve mobility.
• Restore or enhance functional movement and independence.
• Ensure all treatments follow the established plan of care.
• Monitor and document patient progress and response to treatment.
• Educate patients, families, and caregivers on therapy plans and goals.
• Maintain accurate documentation in compliance with state and federal regulations.

Position Overview

As a Physical Therapist, you will evaluate, develop, and implement individualized treatment plans for residents and patients with temporary or long-term physical conditions. You will play a key role in restoring function, promoting healing, and improving overall quality of life.
