Job summary

San Juan Regional Rehabilitation Hospital, part of Lifepoint Health, is seeking a Physical Therapist to join our team at the Farmington location. You will conduct PT assessments, develop treatment plans, provide direct patient care to restore strength, mobility, and function.

You will educate patients and caregivers on home exercise programs, document sessions and outcomes, and collaborate with rehab teams to support discharge planning.

Qualifications

• Graduation from an accredited PT program is required.
• One year rehab experience is preferred.
• Current Physical Therapy license is required (PT).

What We're Looking For

• Education: Graduate of an accredited PT program.
• Experience: One year experience in rehab setting preferred.
• License: Current Physical Therapy, Occupational Therapy, OR Speech Language Pathology License
• Certifications: CPR certification required.
