You will collaborate with physicians, families, and interdisciplinary team members to provide high-quality, patient-centered care.### **Key Responsibilities*** Perform **patient evaluations** and develop individualized treatment plans* Provide therapy to improve **strength, balance, mobility, and overall function*** Monitor patient progress and adjust treatment plans as needed* Maintain accurate and compliant **clinical documentation*** Communicate with physicians, residents, families, and care team members* Participate in **care planning, rehab meetings, and discharge planning*** Conduct **staff education and in-service training*** Supervise **PTAs, aides, and students**### ### **Qualifications*** Degree in **Physical Therapy** (Bachelor’s required; Master’s or Doctorate preferred)* Active, unencumbered **Physical Therapist license in South Carolina (required)*** Must maintain all continuing education and remain in good standing with the State Board* Experience in a **skilled nursing or rehab setting preferred*** Familiarity with **PointClickCare (PCC) and Casamba preferred*** Strong clinical, leadership, and communication skills### ### **Why Join Fleetwood Post Acute?**At Fleetwood, you’ll be part of a **stable, experienced team** that values your expertise and supports your growth.
