This role focuses on helping residents improve **mobility, strength, and independence** while working as part of an interdisciplinary care team.### **Key Responsibilities*** Evaluate residents and develop individualized **treatment plans*** Provide skilled therapy services per **physician orders and care plans*** Document evaluations, treatments, and progress notes in compliance with regulations* Communicate effectively with physicians, nursing staff, and care team members* Supervise **PTAs, aides, and students** in accordance with state practice acts* Participate in care planning and interdisciplinary meetings* Educate residents, families, and caregivers on therapy goals and discharge plans* Conduct **staff training and in-service education*** Maintain compliance with all **state, federal, and payer guidelines**### **Qualifications*** Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)* Active **South Carolina Physical Therapist license** (in good standing)* Experience in **skilled nursing or rehabilitation setting preferred*** Familiarity with **PointClickCare and Casamba** preferred* Strong clinical, documentation, and communication skillsWe are an equal opportunity employer.
