Job summary

Element Health Services is seeking a dedicated Physical Therapist to join our team, delivering rehab services to patients in Independent and Assisted Living Facilities in Florida. The role emphasizes evaluations, care planning, and collaboration with physicians to support patient goals.

Licensed PT in Florida with BLS is required; outpatient/Medicare Part B experience is preferred, with 2–4 years of experience. Join a team that values compassionate, outcome-driven care in a supportive setting.

Qualifications

• Graduation from an accredited Physical Therapy program.
• Active PT license in Florida.
• Current BLS certification.
• Outpatient/Medicare Part B experience preferred.
• 2–4 years of experience preferred.

Responsibilities

• Perform initial and ongoing patient assessments.
• Develop and implement tailored treatment plans in consultation with physicians, adjusting as needed.
• Collaborate with the interdisciplinary care team to optimize patient outcomes.
• Provide education and support to patients and families on therapy programs.
• Communicate patient progress and changes with physicians and care teams.
• Document all visits and progress notes on the same day of service.
