Job summary

East Bay Post Acute in Castro Valley, CA is hiring a Physical Therapist (PT) to join our in-house rehabilitation team. You will help residents regain strength, mobility, balance, and independence with individualized care, working alongside an interdisciplinary team.

We offer competitive hourly pay of $60.00–$65.00+ DOE, full-time, part-time, and PRN opportunities, plus ongoing training, mentorship, and opportunities for leadership development.

Qualifications

• Current and active California Physical Therapist (PT) license.
• Strong clinical assessment and treatment-planning skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of an interdisciplinary team.
• Commitment to compassionate, resident-centered care.
• Strong organizational and documentation skills.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled, evidence-based interventions including therapeutic exercise, gait training, balance activities, functional mobility training, and manual therapy.
• Monitor resident progress and adjust treatment plans to maximize functional outcomes.
• Collaborate with physicians, nursing staff, therapy professionals, and the interdisciplinary care team.
• Educate residents and families on therapeutic exercises, mobility techniques, safety, and discharge recommendations.
• Participate in care planning and discharge planning as appropriate.
• Maintain accurate, timely, and compliant clinical documentation.

Key Responsibilities
