Job summary

Central Gardens Post Acute in San Francisco, CA is seeking a compassionate Physical Therapist (PT) to join our in-house rehabilitation team. We offer Full-Time, Part-Time, and PRN opportunities with flexible scheduling based on your professional and personal needs.

The PT will perform evaluations, develop individualized plans, and provide skilled services to residents, with mentorship and professional development in a collaborative setting.

Qualifications

• Current California Physical Therapist license in good standing.
• New graduates are encouraged to apply.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of an interdisciplinary team.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled physical therapy services based on residents' needs and goals.
• Implement evidence-based interventions to improve strength, mobility, balance, endurance, and functional independence.
• Monitor resident progress and adjust treatment plans as appropriate.
• Educate residents and families regarding therapy goals, exercises, mobility, and safety.
• Collaborate with PTAs and the interdisciplinary care team to support positive clinical outcomes.
• Maintain accurate and timely clinical documentation.
• Promote resident safety, dignity, independence, and quality of life.
• Participate in care planning and interdisciplinary team activities as needed.
