Choice Health At Home seeks a licensed Physical Therapist to lead one-on-one home health care in Aurora, CO. You will supervise PTAs, guide patient care from evaluation through discharge, and exercise clinical autonomy to make informed decisions.

Qualifications

• Successful completion of accredited physical therapy program.
• Valid Physical Therapy license.
• 1 year of home health experience preferred.
• CPR certification.
• Valid Driver’s license.
• Valid Auto Insurance.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform comprehensive evaluations and establish clinical diagnoses.
• Develop and implement individualized treatment plans focused on functional outcomes.
• Directly supervise PTAs and ensure treatments align with the plan of care and regulatory standards.
• Provide ongoing guidance, mentorship, and clinical oversight to PTAs.
• Conduct reassessments and supervisory visits to ensure patient progress and quality outcomes.
• Maintain accurate, compliant documentation for care and PTA services.
• Collaborate with patients, families, and the interdisciplinary team.
• Ensure infection control and patient safety standards are followed.

As a Home Health Physical Therapist, you’ll deliver one-on-one patient care while playing a critical leadership role in supervising and guiding Physical Therapist Assistants (PTAs). Your expertise ensures every patient receives safe, effective, and goal-driven care—every visit, every time.

Requirements

Successful completion of accredited physical therapy program and state licensure exam.

Valid Physical Therapy license

1 year of home health experience preferred
