Job summary

Choice Health at Home in Highland Ranch, CO is seeking a Physical Therapist (PT) to provide skilled, one-on-one care in patients' homes. This full-time role offers schedule autonomy and the chance to build long-term patient relationships.

You'll supervise PTAs, coordinate with physicians, and develop individualized treatment plans aimed at restoring function and independence. CPR certification and a valid license are required, with Home Health experience preferred.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and unrestricted Physical Therapist license.
• Minimum one year of Home Health experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation with current auto insurance.
• Experience with HomeCare HomeBase preferred.

Responsibilities

• Perform comprehensive patient evaluations and establish clinical assessments to guide care planning.
• Develop and implement individualized treatment plans focused on restoring function and achieving measurable outcomes.
• Reassess patient progress and adjust treatment approaches to maximize recovery.
• Deliver evidence-based therapy interventions in the home setting.
• Educate patients and caregivers to promote independence and long-term success.
• Collaborate with physicians and interdisciplinary care teams to coordinate patient care.
• Maintain timely, accurate, and compliant documentation.
• Supervise and mentor Physical Therapist Assistants (PTAs) to support quality outcomes and continuity of care.
• Follow infection prevention and patient safety standards during all visits.

Physical Requirements

• Ability to stand, walk, sit, bend, kneel, squat, and climb for extended periods
• Ability to lift, carry, push, and pull up to 100 pounds as needed for patient care activities
