Choice Health at Home in Boulder, CO is seeking a Physical Therapist (PT) to provide skilled, one-on-one home health care and guide PTAs within an interdisciplinary team. Each visit helps patients progress in a familiar home setting.

You will evaluate, diagnose, and develop individualized plans, reassess progress, ensure safety, document precisely, and collaborate with families to maximize recovery and independence in the patient’s home.

Qualifications

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license.
• 1 year of home health experience preferred.
• CPR certification.
• Valid Driver’s license and auto insurance.
• HomeCare HomeBase experience preferred.

Responsibilities

• Conduct comprehensive evaluations and establish clinical diagnoses to guide care.
• Develop and implement individualized treatment programs focused on real progress and function.
• Reassess patients and modify care plans to maximize recovery and results.
• Apply infection control and safety standards in all patient interactions.

As a Home Health Physical Therapist, you’ll practice at the very top of your license, delivering skilled, one-on-one care directly in patients’ homes. No two days are the same, and every visit is an opportunity to create meaningful progress for someone in their most comfortable environment.

You’ll also lead and support Physical Therapist Assistants (PTAs), ensuring that care is delivered safely, effectively, and aligned with each patient’s goals. Working closely with an interdisciplinary team, you’ll continuously adjust treatment plans to drive the best possible outcomes.

In this role, you will:

Working Hours

To meet the needs of our patients and clients, employee work hours may vary and will be set by each employee’s supervisor, manager, and operating license documentation.

Employees should consult with their supervisors regarding their schedules and time recording procedures. Choice Health at Home, the Company, is open Monday through Friday, office hours

are 8:00 am to 5:00 pm (CST).

Mileage

The Company recognizes the use of personal vehicles by personnel for business purposes as part of their employment. The company will reimburse the eligible employee $0.50 per

mile to cover personal care use, auto insurance, depreciation, and gas. Each employee who is eligible for mileage reimbursement from the Company will be required to track and report mileage

accurately and timely each pay period.
