Outcome HC is seeking a Weekend PRN Physical Therapist to evaluate, plan, and deliver PT services for short-term rehab clients and long-term care residents at King David Nursing and Rehabilitation Center. The role requires independent weekend work and collaboration with an interdisciplinary team.

Experience in SNF/LTC and proficiency with Nethealth and PCC EMR are required; at least one shift per month is expected as part of our flexible PRN schedule.

Job description

The Weekend PRN Physical Therapist assumes responsibility and accountability for evaluating, planning and delivery of appropriate physical therapy services, within a framework of interdisciplinary collaboration and consultation for our short-term rehabilitation clients and long-term care residents at King David Nursing and Rehabilitation Center. Our PRN PT works independently on weekends to perform evaluations and treatments for sub-acute rehab clients preparing for d.c home.

Experience in SNF/LTC required Utilizes Nethealth and PCC EMR. At least one shift per month is required.
