Job summary

Outcome HC is seeking a Weekend PRN Physical Therapist to evaluate, plan, and deliver PT services for short-term rehab clients and long-term care residents at King David Nursing and Rehabilitation Center. The role requires independent weekend work and collaboration with an interdisciplinary team.

Experience in SNF/LTC and proficiency with Nethealth and PCC EMR are required; at least one shift per month is expected as part of our flexible PRN schedule.

Responsibilities

• Evaluate, plan and deliver physical therapy services for sub-acute rehab and long-term care residents.
• Collaborate with interdisciplinary team and provide consultation as needed.
• Work independently on weekends as a PRN PT.
• Experience in SNF/LTC required; uses Nethealth and PCC EMR; at least one shift per month.
