King David Nursing and Rehabilitation Center is seeking a Weekend PRN Physical Therapist to evaluate, plan and deliver physical therapy services for short-term rehab and long-term care residents. The role requires independent weekend coverage and collaboration with an interdisciplinary team.

Experience in SNF/LTC is required; familiarity with Nethealth and PCC EMR is preferred. At least one shift per month is expected to maintain continuity of care.

• Experience in SNF/LTC required.
• Familiarity with Nethealth and PCC EMR.
• Weekend PRN availability (at least one shift per month).

Responsabilités

• Evaluate, plan and deliver PT services for sub-acute rehab and long-term care residents.

The Weekend PRN Physical Therapist assumes responsibility and accountability for evaluating, planning and delivery of appropriate physical therapy services, within a framework of interdisciplinary collaboration and consultation for our short-term rehabilitation clients and long-term care residents at King David Nursing and Rehabilitation Center. Our PRN PT works independently on weekends to perform evaluations and treatments for sub-acute rehab clients preparing for d.c home.

Experience in SNF/LTC required Utilizes Nethealth and PCC EMR. At least one shift per month is required.
