Job description

Summary

The Physical Therapist plans, organizes and implements physical therapy services in accordance with current federal, state and local standards governing the facility and as may be coordinated by the Director of Rehabilitation to ensure that the highest degree of quality care is delivered.

Essential Job Duties and Responsibilities

  1. Screens, evaluates, develops, organizes and implements patient/resident care programs through the utilization of selected physical therapy techniques.
  2. Applies selected muscle, nerve and joint, and functional ability tests to assist physicians and other health personnel in determining treatment diagnosis, prognosis and patient/resident progress.
  3. Evaluates, fits, and adjusts prosthetic and/or orthotic devices and recommends modifications to orthotics or prosthetics.
  4. Participates in conferences, consultations, Facility Care Plan meetings, as needed, and educational programs designed to improve patient/resident care and staff performance as directed by the Director of Rehabilitation.
  5. Instructs patient/resident, family, and/or caregivers in physical therapy recommendations to be continued upon patient/resident discharge.
  6. Consults with physicians concerning goals and treatment plans, and monitors the presence of any contradictions to treatment.
  7. Adhering to regulatory guidelines ensures all physical therapy documentation/treatment conforms to documentation/billing policies and procedures and meets requirements for third party payer documentation and reimbursement.
  8. Participates in quality improvement studies and monitors program evaluation indicators as directed by the Director of Rehabilitation.
  9. Working with the Director of Rehabilitation, provides oversight and direction of the Physical Therapist Assistants, Techs, and Volunteers and assists in the supervision and education of affiliating students.
  10. Assists in the assurance that the physical surroundings and equipment are sanitary, orderly and free from conditions that may cause safety hazards.
  11. Performs other duties as assigned and allowed by State Practice Acts and regulation.
  12. Demonstrates working knowledge of and compliance with the facility Compliance program, policies and procedures, applicable federal and state laws, rules and regulations.
  13. Demonstrates compliance with the protection of patient/resident health information and company sensitive information including compliance with company policies and procedures, applicable federal and state laws, rules, and regulations.
  14. Shall ensure that all employees under their direct supervision receive training in and comply with the facility Compliance program, policies and procedures, applicable federal and state laws, rules and regulations. As a supervisor, must administer appropriate disciplinary action for violations in the above in consultation with the Director of Rehabilitation and facility Administrator.

Other Duties

The Physical Therapist currently supervises Physical Therapist Assistant(s), Rehab Tech(s), and students.

Qualifications

  1. Graduate of an APTA accredited physical therapy school and eligible for licensure in the state in which the program is located.
  2. Prior experience in long-term care is preferred.
  3. Ability to work flexible work hours to support state and federal resident care requirements.

  1. Ability to read, write, speak, and understand English.
  2. Ability to relate positively, effectively and appropriately with patients, families, community members, volunteers and other staff on a daily basis. Possess special interest in, and a positive attitude about, working with long-term care patients and the elderly.
