AlwaysNear Home Care LLC is seeking a dedicated Physical Therapist (PRN) to join our team serving clients in the Atlanta, GA area. Deliver high-quality, patient-centered care directly in clients' homes, with a flexible PRN schedule and meaningful impact on independence and mobility.

Ideal candidate holds a GA PT license, a DPT or equivalent, and has home health or outpatient PT experience. You will collaborate with an interdisciplinary team and document progress in compliance with standards.

Qualifications

• Licensed Physical Therapist in Georgia (GA).
• Experience in home health or outpatient PT preferred.
• Strong communication and patient education skills.
• Ability to work independently with a flexible PRN schedule.

Responsibilities

• Conduct comprehensive physical therapy evaluations for home care patients.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, manual therapy, and neuromuscular re-education.
• Monitor patient progress and adjust care plans as clinically appropriate.
• Educate patients and caregivers on home exercise programs and injury prevention.
• Collaborate with interdisciplinary home health team to ensure coordinated care.
• Maintain accurate and timely clinical documentation in accordance with regulatory standards.

• Excellent communication, interpersonal, and patient education skills
• Reliable transportation and valid driver's license for home visit travel throughout Atlanta, GA
• Ability to work independently and manage a flexible PRN schedule
