Job summary

AlwaysNear Home Care LLC in Atlanta, GA is seeking a dedicated Physical Therapist (PRN) to deliver patient-centered care in clients' homes. You will conduct evaluations, develop treatment plans, and provide therapeutic interventions to restore function and mobility.

Ideal candidates have a GA PT license, DPT or equivalent, and experience in home health or outpatient PT. Strong communication and ability to work independently on a flexible PRN schedule are essential.

Qualifications

• Georgia PT license in good standing.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• Home health or outpatient physical therapy experience preferred.
• Excellent communication, interpersonal, and patient education skills.

Responsibilities

• Conduct comprehensive physical therapy evaluations and assessments for home care patients.
• Develop and implement individualized treatment plans to restore function and improve mobility.
• Provide therapeutic interventions including therapeutic exercise, manual therapy, and neuromuscular re-education.
• Monitor and document patient progress, adjusting care plans as clinically appropriate.
• Educate patients and caregivers on home exercise programs and injury prevention strategies.
• Collaborate with interdisciplinary home health team members to ensure coordinated patient care.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
