AlwaysNear Home Care LLC is seeking a dedicated and compassionate Physical Therapist (PRN) in Atlanta, GA to deliver high-quality, patient-centered care directly in clients' homes. You will evaluate, treat, and support patients in their homes with flexible PRN scheduling.

The role requires a Georgia PT license and a DPT, with home health or outpatient experience preferred. You will collaborate with an interdisciplinary team and educate patients on exercises and safety to promote independence.

Qualifications

• Valid Georgia Physical Therapist (PT) license in good standing.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• Home health or outpatient physical therapy experience preferred.
• Excellent communication, interpersonal, and patient education skills.
• Reliable transportation and valid driver's license for home visit travel throughout Atlanta, GA.
• Ability to work independently and manage a flexible PRN schedule.

Responsibilities

• Conduct home care PT evaluations and assessments.
• Develop and implement individualized treatment plans.
• Provide interventions such as therapeutic exercise and manual therapy.
• Monitor progress and adjust care plans.

• Provide therapeutic interventions including therapeutic exercise, manual therapy, and neuromuscular re-education


• Monitor and document patient progress, adjusting care plans as clinically appropriate


• Educate patients and caregivers on home exercise programs and injury prevention strategies


• Collaborate with interdisciplinary home health team members to ensure coordinated patient care


• Maintain accurate and timely clinical documentation in compliance with regulatory standards
