Aviator Home Health is seeking a skilled Physical Therapist to join a dynamic clinical team delivering high-level home health care. You will perform initial and ongoing assessments, develop care plans with physicians, and supervise PTA/HHA to ensure excellent patient outcomes.

The role emphasizes hands-on therapy, documentation, and adherence to infection control, with a strong support system from office and management staff.

Qualifications

• BS in Physical Therapy or higher and TX licensure as a PT.
• Current Texas State License as a Physical Therapist and Texas Driver’s License.
• Proof of BLS for healthcare workers CPR.
• Home Health experience preferred; HomeCare HomeBase knowledge a plus.

Responsibilities

• Perform initial and ongoing clinical assessments, including OASIS at appropriate times.
• Develop and revise Plan of Care in consultation with physician and care team.
• Provide therapeutic treatments and evaluate equipment needs as applicable.
• Evaluate outcomes and coordinate discharge planning.
• Supervise PTA and HHA as appropriate.
• Complete documentation promptly.

With Aviator, you're not just a clinician out in the field, you'll be part of a CLINICAL TEAM that works together for the betterment of our patients with unparalleled support from our office and management staff.Essential Functions of the Physical TherapistPerform initial and ongoing clinical assessments to identify patient needs, determine level of functioning, including OASIS assessments at appropriate time points and appropriateness of patient for home health.Develop and revise a Plan of Care in consultation with the physician and other care-team members.Provide therapeutic treatments and evaluation of equipment needs as applicable.Evaluate outcomes of care with appropriate discharge planning.Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.Accept clinical assignments that are consistent with education and competence to care for patients.Supervise PTA and HHA as appropriate.Complete appropriate documentation in a timely manner.Meet mandatory continuing education requirements of the agency/licensing board and participate in in-service programs.Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate.Promote Agency philosophy and administrative policies.Demonstrate commitment, professional growth and competencyProficient and comfortable using technology to communicate and perform essential job functions.Job Qualifications of the Physical TherapistEducation: Bachelor of Science in Physical Therapy or higher from an American Physical Therapy Association approved program.Licensure: Current Texas State License as a Physical Therapist, current Texas Driver’s LicenseSkills: Therapy skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current BLS for healthcare workers CPR.
