Job summary

Aviator Home Health is seeking a Speech Language Pathologist to deliver specialized therapy for speech, language, swallowing, and cognitive disorders in home health settings across the Plano, TX area. Qualifications

• Master’s degree or doctorate in Speech-Language Pathology from an accredited college or university.
• Current Texas State License in Speech-Language Pathology.
• Current Texas Driver's License.
• Two years of experience in an acute care, rehabilitation setting, or home health preferred.

Responsibilities

• Perform initial and ongoing clinical assessments to identify patient needs.
• Develop and revise Plan of Care in consultation with the physician and care team.
• Provide therapeutic treatments for speech, language, cognitive and swallowing disorders.
• Evaluate outcomes of care with appropriate discharge planning.
• Supervise speech language assistant and HHA as appropriate.
• Complete appropriate documentation in a timely manner.
• Meet mandatory continuing education requirements of the agency/licensing board.
