Job summary

Aviator Home Health is seeking a skilled Physical Therapist to join our clinical team. Perform initial and ongoing assessments, develop care plans with physicians, and provide therapy with expert use of equipment in patient homes.

We emphasize hands-on patient care and clinical teamwork, with strong support from our office and management staff. Texas licensure and BLS CPR are required, plus reliable transportation for home visits.

Qualifications

• Bachelor of Science in Physical Therapy or higher from an approved program.
• Current Texas State License as a Physical Therapist.
• Proof of current BLS for healthcare workers CPR.
• Knowledge of durable medical equipment (DME).
• Reliable transportation and valid auto liability insurance.

Responsibilities

• Perform initial and ongoing clinical assessments, including OASIS assessments at appropriate time points and appropriateness of patient for home health.
• Develop and revise a Plan of Care in consultation with the physician and other care-team members.
• Provide therapeutic treatments and evaluation of equipment needs as applicable.
• Evaluate outcomes of care with appropriate discharge planning.
• Meet mandatory continuing education requirements of the agency/licensing board and participate in in-service programs.
• Promote Agency philosophy and administrative policies.

Good interpersonal skills. Proof of current BLS for healthcare workers CPR.
