Job summary

Encore Village is seeking a PRN Physical Therapist to join our outpatient departments in Illinois. The therapist will assess needs, develop and deliver rehab programs to facilitate recovery and independence.

Responsibilities include collaborating with other disciplines, educating patients and caregivers, and guiding therapy to promote health and functional outcomes. Illinois licensure and CPR certification are required.

Qualifications

• Graduate of an accredited PT therapy program.
• Current IL State Licensure (PT).
• CPR Certification.
• Proficient with NetHealth therapy documentation.

Responsibilities

• Assess patient needs and develop therapy plans.
• Deliver physical therapy services in outpatient settings.
• Refer to other disciplines to address patient needs.
• Educate patients and caregivers to promote independence.

Encore Village is currently hiring for a PRN Physical Therapist. Looking for our outpatient departments. The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. The Physical Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients. They implement individualized therapy programs designed to restore, reinforce and enhance performance. In additiona, the Physical Therapist instructs, educates, and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, to diminish or correct pathology. They direct activities to promote and maintain health.

Qualifications:

• Graduate of an accredited Physical Therapist (PT) therapy program
• Current IL State Licensure
• CPR Certified
• Proficient with therapy documentation (NetHealth)
