Job summary

Encore Village in Schaumburg, IL is seeking a PRN Physical Therapist to support our outpatient departments. The therapist will assess patients, develop and deliver therapy programs to facilitate rehabilitation.

Qualifications

Responsibilities

• Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation.
• Identify the need for and make referrals to other disciplines to address patient needs.
• Implement individualized therapy programs designed to restore, reinforce and enhance performance.
• Instruct, educate, and train patients and caregivers in skills to promote independence and productivity.
• Direct activities to promote and maintain health.
