You will collaborate with physicians, educate patients and families, maintain documentation, and participate in ongoing professional development and infection-control practices.

Qualifications

• A Doctorate, Masters, or Bachelors in Physical Therapy required.
• Valid PT license and BLS certification required.
• Excellent communication and teamwork skills.
• Customer-service orientation and basic computer skills.

Responsibilities

• Treatment Planning & Care: Implements treatment plans, selects activities, revises goals with patient/family input, provides education, discharge instructions, and referrals.
• Patient Assessment: Collaborates with physicians to establish and evaluate treatment programs, goals, and discharge plans with patient/family involvement.
• Documentation: Maintains accurate records of evaluations, treatments, education, and discharge documentation per departmental protocols.
• Professional Conduct & Customer Service: Upholds ethics, maintains confidentiality, communicates effectively, attends meetings, and promotes teamwork; applies AIDET and infection-control policies.
• Professional Development: Engages in ongoing education, completes required competencies, participates in orientation, and maintains licensure and certifications.
