Job summary

Banner Rehabilitation Hospital – West in Peoria, AZ is seeking a Per Diem Physical Therapist to join our inpatient rehabilitation team. You will perform initial and ongoing patient assessments, develop treatment plans, supervise PT/PTA students, and participate in discharge planning.

Qualifications include state PT licensure and BLS certification (or within 90 days). The role offers a meaningful care environment with potential for advancement within a large rehab network.

Qualifications

  • State Licensure: Hold a current state licensure as a Physical Therapist (PT).
  • Certified BLS: Possess Certified BLS or obtain it within 90 days of employment.

Responsibilities

  • Perform initial and ongoing systematic patient assessment.
  • Promote continuous quality improvement.
  • Teach and counsel patients/families.
  • Set goals and develop treatment plans.
  • Identify and solve patient-specific and facility-wide needs.
  • Participate in discharge planning, including education and adaptive equipment.
  • Supervise PTs/PTA students and therapy staff as needed.
  • Conduct individual patient therapy regimens.
  • Monitor patient responses and modify treatment as needed.
  • Complete documentation per department policies.
  • Participate in ongoing education seminars and in-services.

Responsibilities
  • Performing initial and ongoing systematic patient assessment.

  • Working cooperatively to identify and solve patient-specific and facility-wide needs.
  • Participating in discharge planning for each patient, including placement, patient/family education and adaptive equipment.
  • Supervising Physical Therapy Assistants, as well as supervising PT and PTA students.
  • Conducting individual patient therapy regimens.
  • Monitoring patient's response to treatment and modifying treatment during sessions, as needed.
  • Completing appropriate documentation according to department policies and procedures.
  • Participating in departmental, hospital, and community continuing education seminars and in-services.
Qualifications
