Physical Therapist - In-House SNF Job Position

Physical therapist for an In-house Skilled Nursing Facility

Job Type: Per Diem

Shifts: Weekdays and Weekends

Job Purpose: To assist in planning, organizing, developing, and directing Physical therapist services in accordance with current applicable federal, state, and local standards, guidelines, and regulations. The goal is to ensure the highest quality of resident care, under the direction of the DOR, Administrator, or Regional support team.

Job Functions:

1. Provide Physical therapist services to residents according to established standards, procedures, and productivity standards.
2. Follow company procedures regarding documentation and billing of services.
3. Use sound judgment in evaluating, planning, implementing, and following up on resident therapy programs.
4. Conduct regular screenings to determine the need for intervention/treatment.
5. Evaluate residents to gather data for treatment planning and implementation.
6. Develop treatment plans with clear short and long-term goals.
7. Communicate findings and plans effectively to residents, families, and team members.
8. Complete compliant and medically necessary documentation as per CMS expectations in NetHealth EMR systems.

Education & Requirements:

1. Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
2. Active Physical Therapy license in the state(s) of practice.
3. Ongoing continuing education and professional development to maintain licensure and certification.
