As a Physical Therapist, you will assess, plan, organize, and administer rehabilitative programs to improve mobility, relieve pain, and increase strength for residents. Our therapy team has recently transitioned to an in-house model, and we are seeking a PRN therapist to support our residents' therapy goals and enhance their quality of life.

Job Qualifications

• Graduate of an accredited therapy program
• 2-3 years of related experience preferred, or an equivalent combination of education and experience
• Current Physical Therapist state license
• CPR certification

Responsibilities

• Plan and administer medically prescribed physical therapy to restore function, alleviate pain, and prevent disabilities.
• Assess and interpret evaluations and test results to provide quality care.
• Develop and implement individualized treatment plans with short- and long-term goals.
• Adjust treatment plans based on patient progress.
• Administer manual exercises, massages, and use equipment as necessary.
• Evaluate and record patient progress, modifying treatment as needed.
• Document patient care services according to protocols.
• Communicate progress to patients, caregivers, and healthcare team members.
• Develop post-treatment care plans, including in-home services.
