Job summary

Nexus Health Management is seeking a PRN Physical Therapist to join our Elmwood Park facility. You will assess evaluations, develop plans, and provide patient-centered therapy to enhance function and healing. Collaboration with a multidisciplinary team and ongoing patient education are essential to success.

Illinois license is required, with SNF/long-term care experience preferred. Familiarity with Net Health EDS is a plus, ensuring compliant record-keeping and treatment documentation.

Qualifications

• Active Illinois license in Physical Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Assess patient evaluations and test results.
• Develop treatment plans using various techniques.
• Create personalized rehabilitation programs.
• Administer prescribed physical therapy treatments to relieve pain and restore function.
• Provide clinical support and education to patients and families.
• Collaborate with interdisciplinary team to ensure quality care.
• Ensure timely and accurate documentation per regulations.

PRN Physical Therapist Responsibilities

• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.

PRN Physical Therapist Qualifications
