Job summary

BRIA is seeking a Physical Therapist located in Chicago Heights, Illinois. The role involves assessing patient evaluations, developing treatment plans, and creating personalized rehabilitation programs.

Qualifications

• Active/Valid Illinois license in Physical Therapy is required.
• Experience in SNF/Long term care is preferred.
• Current knowledge of treatment practices necessary.

Responsibilities

• Assess and interpret patient evaluations and test results.
• Develop treatment plans using various techniques.
• Create personalized rehabilitation programs for patients.
• Administer medically prescribed physical therapy treatments.
• Provide clinical support and education to patients and caregivers.
• Collaborate with interdisciplinary team members for quality care.
• Ensure thorough and timely documentation complying with regulations.

Skills

Active/Valid Illinois license in Physical Therapy

Current knowledge of treatment practices

Knowledge of Net Health Electronic Documentation System

Physical Therapist Responsibilities:

• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.

Requirements

Physical Therapist Qualifications:

• Active/Valid Illinois license in Physical Therapy
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
