Job summary

Creative Solutions in HealthCare is seeking a Per Diem Physical Therapist (PRN) to join our in-house rehab team in Texas. You will evaluate patients, develop and adjust treatment plans, and provide rehabilitation services to residents under the plan of care.

Responsibilities include documenting care, collaborating with an IDT, and educating patients and families. New grads are welcome; flexible schedules and competitive compensation are offered.

Qualifications

• Active Texas PT license in good standing.
• SNF/Long-term care experience preferred.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different personalities.

• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain and restore function.
• Ensure all treatment delivered follows the plan of care.
• Monitor patient response and modify treatment with IDT collaboration.
• Provide clinical support and education to patients and caregivers.
• Collaborate with interdisciplinary team to ensure quality care.
• Complete documentation timely per regulations.
• Attend required meetings with Director of Rehab.

Job description

We have an amazing opportunity for a Per Diem (PRN) Physical Therapist! We are looking for a PT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills

Physical Therapist Requirements

• Active/Valid Texas license in Physical Therapy
