Job summary

Arlington Heights Health is seeking a Per Diem (PRN) Physical Therapist to join our in-house rehab team in Texas. The PT will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to improve function and reduce pain in a long-term care setting.

Ideal candidates hold an active Texas Physical Therapy license, have SNF/long-term care experience, and can collaborate with an interdisciplinary team. New grads are welcome; flexible schedules are offered.

Qualifications

• Provide PT in SNF/long-term care setting.
• Develop and adjust treatment plans per plan of care.
• Maintain accurate documentation compliant with regulations.
• Collaborate with rehab team and IDT.

Responsibilities

• Consult with patients to learn about their physical condition.
• Assess patient evaluations and test results.
• Develop treatment plans using appropriate techniques.
• Create personalized rehabilitation programs for patients.
• Administer PT treatments to relieve pain and restore function.
• Ensure all treatment aligns with the plan of care.
• Monitor patient response and adjust as needed with IDT.
• Provide education to patients, families and caregivers.
• Collaborate with interdisciplinary team for quality care.
• Ensure timely documentation compliant with regulations.
• Attend meetings designated by the Director of Rehab.

We have an amazing opportunity for aPer Diem (PRN) Physical Therapist!

We are looking for aPT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy.
• In good standing with all regulatory agencies and licensing boards.
