Job summary

Creative Solutions In HealthCare is seeking a Per Diem Physical Therapist to join our in-house rehab team in a SNF/long-term care setting. The PT will collaborate with residents and the interdisciplinary team to design and execute personalized rehabilitation plans and document care in compliance with state and company policies.

The role emphasizes patient-centered care, flexibility, and the ability to adapt across different patient needs while maintaining high quality standards.

Qualifications

• Experience in SNF/long-term care settings preferred.
• Knowledge of Net Health Electronic Documentation System (EDS) helpful.
• Ability to work with patients who have different personalities.

Responsibilities

• Consult with patients to assess their physical condition.
• Assess evaluations and test results to inform care.
• Develop and implement personalized rehabilitation plans.
• Administer PT treatments to reduce pain and improve function.
• Document care thoroughly to meet regulatory requirements.
• Coordinate with IDT and stakeholders for ongoing care.
• Provide education to patients and families about therapy.

We have an amazing opportunity for a Per Diem (PRN) Physical Therapist!

We are looking for a PT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements

• Active/Valid Texas license in Physical Therapy.
• In good standing with all regulatory agencies and licensing boards.
