Job summary

Creative Solutions In HealthCare is seeking a Per Diem (PRN) Physical Therapist to join our in-house team serving residents with person-centered care. The role involves evaluating patients, designing and implementing rehabilitation plans, and documenting outcomes in coordination with the Rehab team.

The ideal candidate holds an active Texas PT license, has SNF/long-term care experience, and can work flexibly to support patient needs.

Qualifications

• Active Texas PT license in good standing.
• SNF/long-term care experience preferred.
• Familiarity with Net Health EDS is a plus.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using various techniques.
• Creating personalized rehabilitation programs for patients.
• Administering PT treatments to relieve pain and improve function.
• Ensure all treatment aligns with the plan of care.
• Monitor patient response and adjust as needed with IDT input.
• Provide education to patients, families, and caregivers.
• Collaborate with the interdisciplinary team to ensure quality care.
• Ensure thorough and timely documentation per regulations.

Job description

We have an amazing opportunity for a Per Diem (PRN) Physical Therapist! We are looking for a PT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
