Job summary

Carroll County Memorial Hospital is seeking a PRN Physical Therapist to evaluate and treat patients across inpatient, outpatient, and swing bed settings at our Carrollton, KY campus. You will develop evidence-based plans, provide skilled PT interventions, reassess progress, and maintain complete EMR documentation.

Qualifications include a Kentucky PT license, BLS, and a minimum of three years of clinical PT experience; DPT preferred.

Qualifications

• Graduate of an accredited Physical Therapy program.
• DPT preferred; Bachelor’s or Master’s acceptable if licensed.
• Current Kentucky Physical Therapist License.
• Current BLS Certification.
• Minimum three years of clinical physical therapy experience.
• Experience in a Critical Access Hospital or rural setting preferred.
• Experience with outpatient, inpatient, and swing bed rehabilitation preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans using evidence-based practices.
• Provide skilled physical therapy interventions for inpatient, outpatient, swing bed, and other rehabilitation populations.
• Reassess patient progress and modify treatment plans as appropriate.
• Maintain accurate, timely documentation in the electronic medical record.
• Educate patients, families, and caregivers on treatment plans and home exercise programs.
• Collaborate with physicians, nursing, occupational therapy, case management, and other disciplines to coordinate patient care.
