Job summary

Creative Solutions In HealthCare is seeking a Per Diem Physical Therapist to join our in-house rehab team in Texas. The PT will evaluate patient needs, develop and implement rehabilitation plans, and monitor progress within an interdisciplinary setting.

The role supports flexible scheduling and non-contracted rehab services, with opportunities for new grads to apply and competitive compensation. Texas licensure is required to practice.

Qualifications

• SNF/LTC experience preferred.
• Current knowledge of physical therapy treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Consult with patients to learn about their physical condition.
• Assess and interpret patient evaluations and test results.
• Develop treatment plans using various techniques.
• Create personalized rehabilitation programs for patients.
• Administer prescribed physical therapy treatments to relieve pain and restore function.
• Ensure all treatment complies with the care plan.
• Monitor patient response and adjust treatment as needed.
• Provide education to patients, families, and caregivers.
• Collaborate with the interdisciplinary team to ensure quality care.
• Ensure documentation is thorough and timely per regulations.
• Attend meetings designated by Rehab Director.

Job description

We have an amazing opportunity for a Per Diem (PRN) Physical Therapist! We are looking for a PT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills

• SNF/Long term care experience preferred.

Physical Therapist Requirements

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
