Job summary

Creative Solutions In HealthCare is seeking a Per Diem (PRN) Physical Therapist to join our in-house rehab team serving residents in a SNF/long-term care environment in Texas. The PT will evaluate patients, develop and implement personalized rehabilitation plans, and document care in accordance with the plan of care and regulatory requirements.

The ideal candidate holds an active Texas PT license, has experience in SNF/LTC settings, and can collaborate with an interdisciplinary team.

Qualifications

• Active/Valid Texas license in Physical Therapy.
• In good standing with regulatory agencies.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans and personalized rehabilitation programs.
• Administering physical therapy treatments to relieve pain and restore function.
• Ensure all treatment aligns with the plan of care and is well documented.
• Collaborate with the interdisciplinary team to provide high-quality patient care.

We have an amazing opportunity for a Per Diem (PRN) Physical Therapist! We are looking for a PT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
