Job summary

Creative Solutions In HealthCare is seeking a Per Diem (PRN) Physical Therapist to join our in-house rehab team serving residents in a person-centered care model.

The role requires Texas licensure and SNF/long-term care experience is preferred. You will assess patients, develop plans, provide PT treatments, document outcomes, and collaborate with an interdisciplinary team to ensure quality care.

Qualifications

• Licensed Physical Therapist in Texas in good standing.
• Experience in SNF/long-term care preferred.
• Knowledge of current PT treatment practices.
• Familiarity with Net Health EHR systems preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Consult with patients to assess physical condition.
• Develop and adjust treatment plans.
• Administer PT treatments per plan of care.
• Document thoroughly and timely in EHR.
• Collaborate with IDT for quality patient care.
• Provide education to patients, families, and caregivers.
• Attend Rehab Director meetings as required.

Job description

We have an amazing opportunity for a Per Diem (PRN) Physical Therapist! We are looking for a PT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
